Job summary

Always Best Care Senior Services is hiring Certified Home Health Aides (HHA) for per diem/per visit work across Solano County, including Vacaville and surrounding areas. The role involves providing one-on-one in-home care under the supervision of nurses and care managers to help patients stay safe, comfortable, and independent.

Qualifications

• Current California Certified Home Health Aide (HHA) certification.
• CMS-approved Home Health Aide Training and Competency Evaluation Program.
• State-approved Nurse Aide Training Program meeting federal requirements.
• State licensure meeting CMS Home Health Conditions of Participation.

Responsibilities

• Provide personal care and related services per Plan of Care (POC).
• Assist with ADLs and IADLs; maintain patient safety and comfort.
• Assist with bathing, dressing, grooming, hygiene, and mobility.
• Obtain and document vital signs; monitor changes and report to RN.
• Complete accurate documentation for each patient visit; coordinate with clinical team.
