Job summary

Professional Case Management is hiring Personal Care Attendants in Tennessee. The role supports clients at home with personal care, nutrition, and daily living activities, following a supervisor's plan of care.

Flexible shifts include PRN, part-time, and full-time hours to fit your schedule, with emphasis on confidentiality and safety. Qualified applicants should have at least a ninth-grade education, be reliable, and pass required TB testing.

Qualifications

• Able to read and write at a basic to intermediate level.
• Possess emotional and cognitive maturity for client interactions.
• Maintain neat, clean personal appearance and health requirements.
• Demonstrate professional PCP knowledge and skills as required.
• Complete TB skin tests annually and provide proof to HR.

Responsibilities

• Follow plan of care to assist with personal hygiene and safe environment.
• Plan and prepare nutritious meals as directed in the care plan.
• Shop for groceries and supplies when instructed.
• Assist with non-skilled personal care and homemaking tasks.
• Complete daily visit reports with time in/out and submit per guidelines.
• Maintain client confidentiality and report changes to supervisor.
