Job summary

Qualifications

• Completion of the ninth grade; prefer high school graduate.
• Able to read and write consistent with job requirements.
• Maturity to work with clients, families, and agency personnel.
• Neat, clean personal appearance.
• Health status meets client safety requirements and TB testing proof available.
• Reliable transportation to client homes.
• Ability to demonstrate PCP knowledge and skills per plan of care.
• Commitment to ethics, confidentiality, and record-keeping.
• Basic non-skilled personal care and housekeeping tasks.

Responsibilities

• Follow the plan of care prepared by the supervisor with the client.
• Help client maintain personal hygiene.
• Assist in maintaining a healthful, safe environment.
• Plan and prepare nutritious meals; shop for groceries as directed.
• Assist client with non-skilled personal care per scope of practice.
• Maintain client confidentiality and complete visit reports daily.
• Report concerns or condition changes to the supervisor or designee.
• Collaborate with other agency personnel involved in care.
• Follow emergency procedures and document required information.
