Job summary

Midtown Oaks Post Acute in Sacramento, CA seeks a motivated Physical Therapist Assistant (PTA) for a part-time role in our rehabilitation team. You will provide treatments under a licensed PT, help patients regain mobility, and collaborate with a multidisciplinary staff to ensure quality outcomes.

Requirements include a California PTA license and graduation from an accredited PTA program; post-acute rehab experience is preferred.

Qualifications

Current California PTA license.
Graduate of an accredited Physical Therapist Assistant program.
Experience in skilled nursing/post-acute rehab preferred.

Responsibilities

Provide physical therapy treatments under the direction and supervision of a licensed Physical Therapist.
Assist patients in improving mobility, balance, strength, endurance, and functional independence.
Implement individualized treatment plans and therapeutic exercise programs.
Accurately document treatments, patient responses, and progress toward goals.
Collaborate with therapists, nursing staff, physicians, residents, and families to support optimal outcomes.
Participate in discharge planning and continuity of care initiatives.
Maintain compliance with all facility, state, and federal regulations.
