Job summary

Merakey is seeking a part-time Psychiatric Mental Health Nurse Practitioner (PMHNP) to support the Assertive Community Treatment (ACT) team in Carlisle, PA. The role is onsite with community visits, totaling 7.5 hours per week.

The PMHNP will diagnose and manage mental health concerns and work with a multidisciplinary team to provide comprehensive care. Responsibilities include evaluating patients, prescribing therapies, and facilitating hospital admissions when needed.

Qualifications

• Certified Nurse Practitioner, PMHNP licensed in the state of Pennsylvania with prescriptive authority required.
• Minimum one year of experience in community mental health preferred.

Responsibilities

• Conduct psychiatric evaluations to determine medication needs and document in EHR.
• Prescribe pharmacologic therapies and monitor progress with adjustments as needed.
• Facilitate patient admissions to hospitals when required.
• Participate in weekly multidisciplinary team meetings for coordinated treatment plans.
