Providers can work flexible hours between 8AM-8PM, seven days a week.

Optum is seeking a Part-Time Nurse Practitioner to join our Home-based Medical Care team to see patients for 10 hours per week in Henry, MO. Optum is a clinician-led care organization creating a seamless health journey for patients across the care continuum.

Position Highlights & Primary Responsibilities:
  1. Establish and maintain effective relationships with patients, caregivers, legal representatives, IDT members, PCPs, other providers, and facility staff when applicable.
  2. Perform initial comprehensive assessments on newly enrolled patients and provide ongoing care, including visits for condition changes.
  3. Develop and update patient management care plans as needed.
  4. Consult with Physician Leaders and coordinate with PCPs for clinical decisions.
  5. Perform basic episodic nursing procedures such as IV placement, blood draws, injections, Foley catheter care, nasal packing, and basic wound care. Training provided.
  6. Conduct post-discharge visits to ensure patient stability aligns with their care goals.
  7. Participate in HEDIS measure campaigns and quality initiatives to ensure high standards of care.
  8. Participate in clinical after-hours on-call rotations.

Required Qualifications:
  • Certification through AANP or ANCC.
  • Current, unrestricted NP licensure in the state of practice.
  • Unrestricted RN licensure in the state of practice.
  • Reliable transportation; compliance with Optum Motor Vehicle Safety policy if driving.
Preferred Qualifications:
  • 2+ years clinical experience in primary care, home-based medical care, geriatrics, palliative care, emergency/urgent care, or similar.
  • Knowledge of long-term care, community resources, and cost-effective care delivery.
  • Knowledge of CPT, ICD-10, and HCPCS codes.
  • Understanding of clinical standards of care.
  • Awareness of NCQA, CMS guidelines, Medicaid/Medicare contracts, and benefits systems.

The hourly range is $43.17 to $84.95, based on full-time employment and factors like location, education, experience, and certifications. Benefits include comprehensive packages, incentive programs, stock purchase options, and 401k contributions. Benefits are subject to eligibility.
