Job summary

Secure Home Health Care is seeking per-diem nurses (LPN/RN) to conduct home visits in Boston and surrounding areas. The role offers part-time options with potential full-time opportunity later, scheduled based on your availability.

Responsibilities include patient assessments, vital signs, meds administration, wound care, care coordination, and patient education. Licensure and CPR are required, with travel to multiple patients per day expected.

Qualifications

• State RN/LPN license is required.
• CPR certificate required.
• 1+ years’ experience in Home Care/VNA, inpatient, or critical/palliative care.
• Ability to travel to multiple patients per day is required.
• Excellent communication, documentation and time-management skills.

Responsibilities

• Assess patients’ condition during every visit and chart observations.
• Perform evaluation tasks, including vital signs and medication review.
• Administer medication as prescribed by the patient’s physician.
• Dress or redress wounds and assess healing progress.
• Educate patients and families on home health care strategies.
• Coordinate with other care providers in the care plan.
• Listen to patient concerns and respond accordingly.
