Job summary

At Peace Health Care Agency is seeking a compassionate Direct Support Professional to empower individuals with disabilities in daily living and community activities. You will provide person-centered care, promote independence, and safeguard dignity and rights in every interaction.

You will support mobility, personal care, and transportation to medical and therapy appointments, while implementing individualized care plans and maintaining HIPAA compliance.

Qualifications

Experience with behavioral support plans (BSPs) and home care practices.
Ability to assist individuals with mobility and personal care needs.
Proficiency in tracking goals and implementing support strategies.
Strong understanding of individual rights and person-centered advocacy.
HIPAA compliance and maintaining confidentiality.

Responsibilities

Deliver hands-on support with daily living activities and mobility.
Foster independence and community involvement for individuals with disabilities.
Implement and adapt individualized care plans and BSPs.
Ensure a safe, respectful environment protecting rights and well-being.
Support individuals with Autism Spectrum Disorder and diverse needs.
Provide transportation and accompany to appointments.
