Job summary

VNA of Florida is seeking an Occupational Therapist to provide home health services in Ft. Pierce and surrounding areas. The role offers flexible scheduling and the chance to make meaningful patient impact within a trusted nonprofit with 50 years of service.

Responsibilities include OT evaluations, treatment planning, improving independence, and educating patients and caregivers while coordinating with interdisciplinary teams. A Florida OT license is required, with HCHB experience preferred.

Qualifications

• Active Florida OT license is required.
• Home health experience preferred.
• HCHB experience preferred.
• Strong clinical reasoning skills.

Responsibilities

• Perform occupational therapy evaluations.
• Develop individualized treatment plans.
• Improve patient independence and safety.
• Assess ADLs and functional limitations.
• Educate caregivers and patients; coordinate care with interdisciplinary teams.
