Job summary

A Path of Care Home Health in Weatherford, OK is seeking a PRN Registered Nurse (RN) on‑call to provide skilled nursing visits in patients' homes. You will assess needs, develop care plans, perform treatments, and educate families, coordinating with physicians to ensure continuity of care.

Qualifications

• Graduate of an approved school of professional nursing.
• Active RN license in Oklahoma.
• At least 1 year of nursing experience.
• CPR certification or ability to obtain within 60 days of hire.
• Valid driver's license and reliable transportation.
• Ability to travel long distances and adapt to changing schedules.
• Home health, critical care, or emergency room experience preferred.
• Strong communication skills, calm under pressure, organized, and multitask.

Responsibilities

• Conduct patient visits to assess nursing needs.
• Develop individualized plans of care from initial evaluations.
• Perform ongoing assessments and revise care plans as needed.
• Provide skilled nursing treatments and educate patients/caregivers on disease processes and self-care.
• Initiate preventive and rehabilitative nursing procedures as ordered.
• Prepare clinical and progress notes and coordinate services.
• Communicate with physicians and interdisciplinary staff to ensure continuity of care.
• Re-evaluate patient needs and assist with discharge planning.
• Supervise LPNs, vocational nurses, and home health aides as needed.
• Practice infection control and report signs of abuse.
• Assist with office tasks and orientation for new nurses.
