HealthBar in South Bend, IN is seeking a part-time Nurse Practitioner to join an employer-sponsored health clinic. The role emphasizes prevention, relationship-based care, and work-life balance, with a flexible weekday schedule and a defined patient population.

You will provide proactive, long-term care across in-person and occupational health settings, focusing on preventive health, chronic disease management, and accessible healthcare for employees and their families.

Qualifications

• Master's degree in Nursing from an accredited program.
• Active, unrestricted FNP certification (AANP or ANCC).
• BLS certification (AHA or Red Cross) within 90 days of hire.
• Must be authorized to work in the U.S. without employer sponsorship.

Responsibilities

• Educate patients on prevention, self-management, and treatment of conditions.
• Provide proactive, relationship-based care focused on prevention and long-term health.
• Coordinate follow-up care and care management outreach.
• Counsel patients on medications, lifestyle interventions, and interactions.
• Order and interpret diagnostic tests including labs and imaging.
• Develop individualized treatment plans with a focus on safety and accessibility.
