Job summary

HealthBar in the South Bend area is seeking a Nurse Practitioner (NP or CNS) for a 24 hours/week, part-time position starting at $55/hour. This employer-sponsored health clinic focuses on prevention, accessibility, and whole-person care, with a patient population that remains consistent.

Qualifications

• Master's degree in Nursing from an accredited program.
• Active, unrestricted FNP certification (AANP or ANCC).
• BLS certification within 90 days of hire and ability to work in the U.S. without sponsorship.

Responsibilities

• Educate patients on prevention, self-management, and treatment of acute and chronic conditions based on individual health needs, goals, and lifestyle.
• Provide proactive, relationship-based care focused on prevention, early intervention, and long-term health outcomes.
• Coordinate follow-up care, care management outreach, and continuity of care services to support patient progress and health goals.
• Counsel patients on medications, supplements, lifestyle interventions, and potential side effects or interactions.
