Qualifications

• Master's degree in Nursing from an accredited program.
• Active, unrestricted FNP certification (AANP or ANCC).
• BLS certification (AHA or Red Cross) within 90 days of hire.
• Must be authorized to work in the U.S. without employer sponsorship.

Responsibilities

• Educate patients on prevention, self-management and treatment of acute and chronic conditions.
• Provide proactive, relationship-based care focused on prevention and long-term health outcomes.
• Coordinate follow-up care and care management outreach to support patient progress.
• Prescribe medications and therapies in accordance with safety and prescribing standards.
• Perform primary care and occupational health procedures including wound care and immunizations.

Education
