Job summary

HealthBar is seeking a Nurse Practitioner (NP or CNS) for a 24-hour-per-week, benefits-eligible role in Indiana. The clinic focuses on prevention, accessibility, and whole-person care outside traditional insurance models, with strong clinical autonomy.

You will practice in a health-clinic setting serving a defined patient population, collaborating with an experienced team to deliver proactive, high-quality care and health optimization for employees and their families.

Qualifications

• Master's degree in Nursing from an accredited program.
• Active Indiana RN license.
• Active, unrestricted FNP certification (AANP or ANCC).
• Active Indiana NP Specialty Certification.
• BLS certification (AHA or Red Cross) within 90 days of hire.
• Must be authorized to work in the U.S. without employer sponsorship.

Responsibilities

• Educate patients on prevention, self-management, and treatment of acute and chronic conditions.
• Provide proactive, relationship-based care focused on prevention and long-term health outcomes.
• Coordinate follow-up care and care management outreach.
• Counsel patients on medications, supplements, and lifestyle interventions.
• Order and interpret diagnostic testing including labs and imaging.
• Develop individualized treatment plans based on history and determinants of health.
• Diagnose and manage acute illnesses and common chronic conditions in various settings.
• Collaborate with specialists and employer teams to improve outcomes.
