Job summary

Hueman is actively seeking Nurse Practitioners to complete in-home Health Risk Assessments for Medicare and Medicaid members near Davison, MI. This role offers flexible scheduling and the chance to support the community through non-clinical, in-home assessments.

Qualified NPs will hold an active State license, national board certification, and BLS. Travel to patients’ homes is required, with comprehensive training provided.

Qualifications

• Master’s Degree or commensurate experience required.
• Active State NP license.
• Active National board certification.
• Current BLS certification.
• 12 months of experience as a paid advanced clinical provider preferred but not required.

Responsibilities

• Travel to and from patients’ homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate and timely documentation.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventative healthcare measures.
