Qualifications

Responsibilities

• Perform wound care as ordered and basic LPN nursing duties.
• Take and document vital signs.
• Assist with/ administer medications as ordered.
• Monitor patient condition and report changes.
• Maintain Plan of Care and physician orders.
• Complete accurate nursing documentation after each visit.
• Communicate with patient, family, and supervisor as needed.
• Maintain patient safety and infection-control procedures.
