Qualifications

• Graduated from a National League of Nursing accredited practical or vocational school of nursing.
• Unencumbered Wisconsin Practical Nurse license.
• 1 year nursing experience preferred.
• Must read, write, speak, and understand English.
• Licensed driver with reliable transportation.

Responsibilities

• Provide direct patient care as defined under the state Nurse Practice Act.
• Implement plan of care and treatment initiated by the registered nurse.
• Provide accurate, detailed, and timely documentation.
• Conduct ongoing assessments.
• Assist with discharge or transfer and educate clients and families.
• Perform wound care and administer medications per nursing plan.
• Observe and report adverse reactions and coordinate with other disciplines.
• Follow infection control principles and agency policies.
