Qualifications

• Must have a valid LPN license in Connecticut.

Responsibilities

• Support residents' diverse needs in post-acute facilities.
• Monitor residents' mental and physical conditions.
• Facilitate intake process and medical documentation.
• Assist with medication verification and monitoring.
• Maintain accurate medical records and charts.
• Help with immunizations, IVs, and blood draws.
• Ensure a comfortable and safe environment for residents.
• Adhere to care plans and compliance regulations.
• Assist with new hire training and orientation.

Skills

Current Connecticut state license to practice as a Licensed Practical Nurse (LPN)

Minimum of 6 months experience as an LPN or related role

Ability to lift at least 50 lbs

Education

Completion of an approved Licensed Practical Nurse (LPN) educational program

Completion of high school or GED/equivalent program
