Job summary

Source One Management Services, LLC is seeking qualified Nurse Practitioners and Physician Assistants for flexible PRN Health Risk Assessments in-home in the Wabash, IN area. You’ll assess Medicare Advantage members, document findings on a user-friendly EMR, and travel locally within your county.

This role offers an 8‑day monthly minimum, competitive pay with incentives, no on-call shifts, and malpractice insurance plus collaborating physicians.

Qualifications

• Active NP or PA license (excluding Pediatrics, PMHNP and Women’s Health specialties)
• Minimum availability of eight (8) full 7.5-hour days per month
• Willingness to travel within roughly 60 minutes of your home

Responsibilities

• Complete comprehensive in-home HRAs for Medicare Advantage patients
• Review each patient’s medical history, current medications, lifestyle, and key social factors
• Conduct basic physical and mental health evaluations
• Travel locally to patients in your county and nearby areas
