Job summary

Source One Management Services, LLC is seeking Nurse Practitioners and Physician Assistants for flexible PRN Health Risk Assessments. Conduct in-home HRAs for Medicare Advantage members, reviewing medical histories and medications, and performing basic physical and mental health evaluations.

Travel to patients within your county and nearby areas using a company-provided iPad. This role offers autonomy, a flexible 8-day per month minimum, competitive pay with incentives, and malpractice insurance

Qualifications

• Active NP or PA license (excluding Pediatrics, PMHNP and Women’s Health specialties)
• Willingness to travel within roughly 60 minutes of your home
• Minimum availability of eight (8) full 7.5-hour days per month

Responsibilities

• Complete comprehensive in-home HRAs for Medicare Advantage patients
• Review patient medical history, medications, lifestyle, and social factors
• Conduct basic physical and mental health evaluations
• Travel locally to patients in your county and nearby areas
