Job summary

Care Planning Institute, Inc. is seeking individuals to perform in-home assessments for elderly clients in the Sioux Falls, SD region. Assessments take 30–60 minutes, with travel time paid at $40/hour, and flexible scheduling from 8am–8pm, including weekends.

We require experience with assessments and will train on the job. TB skin tests are required if not on file. Assessments must be completed within 3–5 days of notification to determine weekly care hours.

Qualifications

• Experience with assessments required.
• Willing to complete TB skin test if not on file.

Responsibilities

• Conduct in-home assessments of elderly clients within a 2-hour drive of Sioux Falls, SD.
• Interview clients and document non-medical care needs in healthcare assessments.
• Complete assessment forms in 30–60 minutes; travel time paid at $40/hour.
