Job summary

New Directions Youth & Family Services, Inc. is seeking a Health Related Social Needs (HRSN) Navigator to provide screenings and navigation services in the WNY or Finger Lakes regions.

The role emphasizes a holistic, strength-based, trauma-informed, and person-centered approach in home and community settings. The Navigator will coordinate a range of services, document progress in the organizations IT platforms, and ensure referrals are triaged and connected to appropriate providers.

Qualifications

• Bachelors degree in Human Services or related field + 2 years of relevant experience
• OR High School Diploma + 4 years of relevant experience
• Must have a valid drivers license and meet driving criteria
• Compliance with agency safety standards and healthy work environment practices

Responsibilities

• Provide empathetic, culturally competent screening for social determinants of health in person, telephonically, or virtually
• Process screenings and referrals with documentation and eligibility verification
• Coordinate across community organizations to connect clients with services
• Document interactions and progress in IT platforms per program requirements
• Conduct telephonic, virtual, or in-home assessments with community members
• Maintain effective communication with clients and referring organizations to finalize referrals
• Refer eligible youth (01) to Medicaid health homes and related programs
• Attend meetings with the cross-sector care network
• Document within IT and transportation platforms within required timeframes
• Maintain 120 billable hours per month
• Support members with questions about needs and services
• Perform additional duties as assigned by supervisors
