Job summary

Talent Solutions Partners is seeking a dedicated Registered Nurse (RN) to join our home health team in Casper, WY. The RN will provide in-home care to seniors, following physician orders and coordinating with families and the care team.

The role requires an active Wyoming RN license and flexibility for full-time or PRN shifts, including evenings and weekends, with mileage reimbursement for travel.

Qualifications

• Current RN license in the state of Wyoming.
• Ability to work full-time and PRN shifts in home health care.
• Strong communication with patients, families and care team.

Responsibilities

• Provide skilled nursing care to seniors in their homes, assessing their physical, emotional, and social needs
• Develop and implement individualized care plans in collaboration with patients, families, and interdisciplinary team members
• Administer medications, treatments, and therapies as prescribed by physicians, and monitor patient responses
• Educate patients and families on disease management, self-care, and wellness strategies
• Collaborate with other healthcare professionals to ensure comprehensive care and coordination of services
• Maintain accurate and detailed records of patient interactions, assessments, and care plans
