Job summary

Adara Home Healthcare is seeking a compassionate Speech Language Pathologist (SLP) to provide skilled home health services in patients' homes across the assigned territory. You will evaluate and treat communication, cognitive, language, voice, and swallowing disorders, collaborating with physicians, caregivers, and the interdisciplinary team to maximize outcomes.

The role is in-person, field-based with flexible scheduling and opportunities for full-time, part-time, or PRN work.

Qualifications

• Master's Degree in Speech Language Pathology from an accredited program.
• Current Colorado SLP license.
• CCC-SLP certification preferred.
• CPR certification or ability to obtain upon hire.
• Valid driver's license and reliable transportation.
• Ability to work independently and exercise sound clinical judgment.
• Preferred: Home Health experience.
• OASIS experience.
• Two or more years of Speech Therapy experience.
• Experience with adult and geriatric populations.

Responsibilities

• Complete speech therapy evaluations and ongoing reassessments.
• Develop individualized treatment plans based on physician orders and patient needs.
• Assess and treat speech, language, cognitive, voice, and swallowing disorders.
• Evaluate patients for dysphagia and provide swallowing therapy as appropriate.
• Establish measurable treatment goals and monitor patient progress.
• Educate patients, families, and caregivers regarding communication strategies and swallowing precautions.
• Collaborate with physicians, nurses, therapists, case managers, and interdisciplinary team members.
• Complete and submit evaluations, progress notes, reassessments, recertifications, discharge summaries and other required documentation.
