Concierge Home Care in Sarasota, FL is seeking a Home Health Speech-Language Pathologist (SLP) to provide skilled therapy in patients’ homes, helping improve communication, cognition, and swallowing. Qualifications

• Active, unencumbered Florida Speech-Language Pathologist license.
• Valid driver’s license with reliable transportation and auto insurance.
• Home health experience preferred but not required.

Responsibilities

• Conduct comprehensive speech, language, and swallowing evaluations.
• Develop individualized treatment plans based on physician orders.
• Provide therapy focused on communication, cognition, and swallowing disorders.
• Educate patients and caregivers on techniques to improve functional outcomes.
• Collaborate with physicians and interdisciplinary care teams.
• Complete Medicare OASIS documentation as required.
• Document visits efficiently using AI-assisted tools.
