The role includes performing patient assessments, developing individualized care plans, coordinating with physicians, and providing skilled nursing care and education, supported by AI-assisted charting and WellSky/Kinnser EMR.

Qualifications

• Active, unencumbered Florida Registered Nurse (RN) license
• Valid driver’s license, reliable transportation, and auto insurance
• Home health experience preferred, but not required

Responsibilities

• Perform patient assessments and develop individualized care plans
• Provide skilled nursing care and patient education
• Coordinate care with physicians and interdisciplinary teams
• Monitor patient progress and adjust care plans as needed
• Complete documentation using stream lined, AI-assisted tools
