Concierge Home Care is hiring Home Health Registered Nurses (RN) to provide 1:1 in-home care in Jacksonville, FL. The role offers a flexible schedule, autonomy, and the potential to earn $85K+ per year with pay-per-visit compensation.

As an RN, you will perform patient assessments, develop care plans, and coordinate with physicians while using AI-assisted documentation tools. Health Insurance starts after 30 days and PTO accrues from day one.

Qualifications

• Active Florida RN license
• Valid driver’s license with reliable transportation
• Home health experience preferred but not required

Responsibilities

• Perform patient assessments and develop individualized care plans
• Provide skilled nursing care and patient education
• Coordinate care with physicians and interdisciplinary teams
• Monitor patient progress and adjust care plans as needed
• Complete documentation using AI-assisted tools
