Job summary

AdvisaCare is seeking a licensed Physical Therapist in Michigan to provide patient-centered home health services within the community. You will develop personalized treatment plans, guide patients through therapeutic exercises, and monitor progress to maximize independence and mobility.

Qualifications

Must hold a valid Physical Therapist license in Michigan.
Minimum of one year home health care experience preferred.
Strong communication skills with patients and families.
Compassionate, patient-centered caregiver.
Ability to work autonomously and manage own schedule.
Reliable vehicle and valid driver's license for patient visits.

Responsibilities

Develop and implement individualized treatment plans to improve mobility and function.
Administer therapeutic exercises and interventions to restore independence and reduce pain.
Assess and document patient progress, adjusting plans as needed.
Collaborate with interdisciplinary care teams to ensure continuity of care.
Educate patients, families, and caregivers on exercises and safety.
Maintain accurate documentation and compliant with standards.
