Qualifications

• Active Physical Therapist license in Texas (TX)
• Professional liability insurance coverage specific to TX license
• Graduate degree or higher in Physical Therapy from an accredited program
• Experience in home health or rehabilitative settings preferred
• Strong ability to adapt care approaches to diverse patients and needs
• Up-to-date knowledge of evidence-based treatment techniques and best practices
• Excellent communication skills and a friendly, professional demeanor with patients

Responsibilities

• Evaluate patients’ physical abilities and functional limitations in a home health setting
• Develop and implement individualized treatment plans to restore function and mobility
• Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining
• Educate patients and caregivers on home exercise programs and safety techniques
• Monitor patient progress and adjust plans to achieve optimal outcomes
• Accurately document evaluations, treatment sessions, and progress using digital platforms
• Collaborate with physicians, nurses, and other therapists
• Conduct home safety assessments and recommend fall prevention/re-injury strategies
• Deliver compassionate care while empowering patients toward independence
