Continuum Therapy Partners, LLC is seeking an Occupational Therapist for our Marshfield, Wisconsin home health team. We offer a flexible schedule with full-time and PRN opportunities, allowing autonomy and meaningful one-on-one care in patients' homes.

Competenze

• Master’s or Doctorate Degree in Occupational Therapy required.
• NBCOT Certification required.
• 1 year of previous experience as an Occupational Therapist preferred.
• Current Wisconsin license/certification

Mansioni

• Evaluates and develops individualized treatment plans to improve patients’ functional independence
• Documents patient progress and modifies treatment plans based on clinical outcomes
• Educates patients and families on adaptive equipment and home safety recommendations
