Job summary

Continuum Therapy Partners, LLC is seeking a Home Health Occupational Therapist in Sheboygan, WI and surrounding areas. You will deliver one-on-one, high-quality care with a flexible PRN schedule and support from a therapist-led team.

Qualified candidates hold a Master’s/Doctorate in Occupational Therapy, a current Wisconsin license, and NBCOT certification. Prior OT experience is preferred, with emphasis on patient safety and functional independence.

Qualifications

• Master’s or Doctorate Degree in Occupational Therapy required.
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required.
• 1 year of previous experience as an Occupational Therapist preferred.

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients’ functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.
