Job summary

JGS Lifecare Corporation's Legacy Lifecare VNA/Hospice is seeking a licensed Speech-Language Pathologist (MA) to evaluate patients for speech and language disorders, initiate assessments on referral, and develop individualized care plans with the physician and multidisciplinary team.

The role provides rehabilitative therapy, documents progress, coordinates with the care team, and participates in case conferences and ongoing professional development.

Qualifications

• Active, valid MA Speech-Language Pathologist license.
• 1–3 years experience in geriatrics, long-term care, or short-term rehab.
• Excellent English communication skills, both written and spoken.

Responsibilities

• Evaluate patients for speech and language disorders and initiate assessments on referral.
• Develop individualized plans of care with physician and care team.
• Provide rehabilitative speech-language therapy and document progress.
• Coordinate care, consult with supervisors, attend case conferences.
• Educate patients and families, support independence and continuity of care.
• Maintain compliance with policies and documentation systems.
