Job summary

Country Manor Campus LLC is seeking a dedicated Home Health Aide to provide compassionate, hands‑on personal care to clients in their homes. This part‑time, day shift role supports clients under the supervision of an RN, promoting comfort, dignity, independence, and quality of life.

Responsibilities include implementing care plans, helping with bathing, grooming, mobility, light housekeeping, and documenting visits.

Qualifications

• Must be at least 18 years of age.
• Must be a Certified Nursing Assistant (CNA) in good standing and current on the Minnesota Nursing Assistant Registry.
• Dependable, punctual, trustworthy, and self-motivated.
• Genuine concern for the well‑being of clients.
• Able to work independently and follow care plans, policies, and procedures.
• Strong communication and interpersonal skills.
• Able to work cooperatively with clients, families, coworkers, and supervisors.
• Dependable transportation to clients' homes.
• Able to safely perform required physical tasks and use proper body mechanics.

Responsibilities

• Review and follow each client's written care plan.
• Provide personal care, including bathing, grooming, oral hygiene, skin care, and toileting.
• Instruct or assist clients with transfers and walking.
• Perform light housekeeping tasks.
• Observe and report changes in client condition or safety concerns to the RN.
• Immediately report any vulnerable adult situation (physical/verbal abuse, neglect, or unsafe living conditions).
• Accurately complete documentation after each client visit.
• Follow infection control, safety, and home care policies and procedures.
• Participate in staff meetings, training, and care planning.
• Maintain professional appearance and conduct.
• Perform other duties as assigned.
