Job summary

BrightStar Care Northwest Compassionate Care, LLC in Brooklyn Park, MN is seeking CNAs for afternoon shifts in home care. Flexible scheduling supports cross-coverage with various client locations, including Maple Grove, Osseo, Champlin, and surrounding areas.

The role involves assisting with daily living activities, monitoring vitals under RN supervision, and documenting changes while delivering compassionate, one-on-one care in patients' homes.

Qualifications

• Current MN CNA certification (preferred) with at least 1 year of home health/assisted living experience.
• Must hold a valid driver's license and have a clean driving record.
• Excellent communication and empathetic bedside manner required.

Responsibilities

• Drive to client homes and provide personal care and daily living assistance.
• Monitor vitals and support RN assessments; report changes promptly.
• Follow care plans, assist with meals, housekeeping, and errands.
• Provide compassionate companionship to enhance well-being.
