Qualifications

• Must hold CDR credentialed status and Dietetic licensure.
• Positive attitude and client-centered approach are essential.
• Excellent communication skills with patients and interdisciplinary teams.

Responsibilities

• Complete clinical nutrition assessments.
• Document using the Nutrition Care Process in the EMR system.
• Collaborate with the interdisciplinary care plan team.
• Complete the MDS - Section K.
• Calculate TF.
• Make recommendations for wounds and significant weight changes.
