Job summary

Concierge Home Care in Ft. Myers, FL is seeking a Home Health Speech-Language Pathologist (SLP) for a flexible PRN role with per-visit pay. You will perform 1:1 patient care in the home, conducting evaluations and developing treatment plans while maintaining autonomy and a strong focus on patient outcomes.

Coverage area includes Ft. Myers with clustered scheduling to minimize drive time. Benefits include mileage reimbursement, referral bonuses, and ongoing clinical support.

Qualifications

• Active, unrestricted Florida SLP license.
• Valid driver’s license and reliable transportation.
• Home health experience preferred but not required.
• OASIS/Medicare documentation experience preferred.

Responsibilities

• Conduct speech, language, cognitive, and swallowing evaluations.
• Develop individualized treatment plans based on physician orders.
• Provide therapy for communication, cognition, and dysphagia.
• Educate patients and caregivers to improve functional outcomes.
• Collaborate with physicians and interdisciplinary teams.
• Complete OASIS documentation as required.
• Document visits using AI-assisted technology.
