Job summary

Concierge Home Care in Crestview, FL is hiring a Home Health Speech-Language Pathologist (SLP) to deliver 1:1 care in patients' homes with a flexible schedule and autonomy. The role covers Crestview and Okaloosa County, offering ability to control income through per-visit compensation and optional PRN or contract arrangements.

You will evaluate speech, language, cognitive, and swallowing functions; develop individualized treatment plans; provide therapy to improve communication and dysphagia;

Qualifications

• Active Florida Speech-Language Pathologist license.
• Valid driver’s license and reliable transportation.
• Home health experience preferred but not required.
• OASIS/Medicare documentation experience preferred.

Responsibilities

• Conduct speech, language, cognitive, and swallowing evaluations.
• Develop individualized treatment plans based on physician orders.
• Provide therapy for communication, cognition, and dysphagia.
• Educate patients and caregivers to improve functional outcomes.
• Collaborate with physicians and interdisciplinary teams.
• Complete OASIS documentation as required.
• Document visits using streamlined, AI-assisted technology.
