ChenMed is seeking a mission-driven Nurse Practitioner (Flex NP) to join our team in a part-time role designed to strengthen care delivery across our centers. The Flex NP focuses on improving same-day access, stabilizing high-risk patients, and supporting care teams without managing a patient panel.

This role emphasizes collaboration, autonomy within teams, and genuine patient relationships. Primarily assigned to the West Philadelphia hub with periodic travel to regional centers as needed.

Qualificações

• ARNP or similar advanced degree in Nursing is required.
• Current Nurse Practitioner Certification in the State of practice is required.
• Board certification by AANP or ANCC is preferred but may be required for certain States.
• Current DEA number from the DEA may be required based on State of practice.
• BLS certification from the American Heart Association or American Red Cross required within first 90 days.

Responsabilidades

• Deliver comprehensive, patient-centered primary care aligned with value-based care model.
• Provide seamless coverage across providers and centers for consistent access to care.
• Support same-day visits, follow-ups, walk-ins, hospital discharges, and careline escalations to maintain continuity.
• Collaborate with providers and care teams to reinforce individualized care plans.
• Build meaningful connections with patients to reinforce trust in their care team.
