Job summary

Molina Healthcare is seeking a Family Care professional to support care management and LTSS coordination in Wisconsin. You will collaborate with a multidisciplinary team to deliver integrated member care and ensure progress toward outcomes.

Qualifications

• At least 2 years health care experience including LTSS exposure.
• Licensure/certification required only if state contract mandates.
• Valid driver''s license and ability to travel for field work.

Responsibilities

• Complete comprehensive member assessments within regulated timelines.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, caregivers, physicians and others.
• Monitor care plan effectiveness and adjust as needed.
• Promote integration of LTSS and behavioral health services.
• Assess medical necessity and authorize waiver services.
• Coordinate ICT meetings for approvals or denials.
