Qualifications

• Has at least 2 years in health care, LTSS/CARE management, or related field.
• Licensure or certification only if required by state contract or regulation.
• Valid driver's license and reliable transportation for travel requirements.
• Experience with diverse populations and high-pressure settings.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including home visits.
• Coordinate waiver enrollment and disenrollment processes for members.
• Develop and monitor care plans with members, caregivers, and health professionals.
• Monitor and document progress, adjust plans to achieve goals.
• Promote service integration across behavioral health, LTSS, and community resources.
