Job summary

Phoenix Home Care & Hospice is seeking a Field Supervisor to lead our at-home care team and ensure safe, high-quality care for clients. You will coach caregivers, onboard new DMH clients, and step in to cover shifts as needed.

You’ll collaborate with the Program Manager to coach staff, perform monthly quality checks, resolve client concerns, and keep care plans and policies on track. A reliable vehicle and dependable communication are essential.

Qualifications

• High school diploma or equivalent, some college a plus.

• Evaluate, coach, and celebrate our caregivers alongside the Program Manager
• Conduct monthly quality checks and help resolve client concerns
