Job summary

Phoenix Home Care and Hospice is seeking a Field Supervisor to lead with heart and ensure safe, high-quality home care for DMH clients. You’ll coach caregivers, support the Program Manager, and jump in to help when it matters most.

You will onboard new DMH clients, train Personal Assistants, conduct quality checks, and address client concerns while supporting the office and keeping care plans on track. A high school diploma and reliable transportation are required, along with a caring attitude

Qualifications

• High school diploma or equivalent.
• Reliable transportation and a working phone.
• A caring attitude and a knack for leading by example.

Responsibilities

• Onboard new DMH clients and train new Personal Assistants (2 hours each).
• Evaluate, coach, and celebrate our caregivers alongside the Program Manager.
• Conduct monthly quality checks and help resolve client concerns.
• Provide hands-on DMH shift coverage when needed.
• Support the office and keep care plans and policies on track.
