Job summary

Halifax Health is seeking a Day Social Worker - Hospice in Florida. Requires an MSW from a CSWE-accredited program and FL licensing (or eligibility). The role involves psychosocial assessments, crisis intervention, and coordinating hospice care for pediatric through geriatric patients.

You will work within an interdisciplinary team, build relationships with patients, families and staff, and help facilitate referrals and transfers to ensure compassionate end-of-life care.

Qualifications

• Must have MSW from a CSWE-accredited school.
• Florida licensed or licensed eligible as a Clinical Social Worker.
• Valid driver’s license with good driving record.
• Minimum 1 year of healthcare experience.
• Social work/counseling background with knowledge of community resources.
• 1 year of social work experience in a healthcare setting.
• Strong clinical and assessment skills including crisis intervention.
• Excellent organization, time-management and self-awareness.
• Customer-service oriented with strong communication and relationship-building.
• Professional interpersonal and written communication with colleagues, physicians and staff.
• Skilled in caring for pediatric, adolescent, adult and geriatric patients.
• Provide and document psychosocial assessments.
• Provide psychosocial support and services to patient/caregiver.
• Facilitate community referrals and Nursing Home/ALF transfers.
• Assess bereavement needs and provide crisis intervention.
• Serve as resource to interdisciplinary team.
• Participate in community education about Hospice care.
