Job summary

PrideGlobal is seeking a Registered Nurse (RN) Intake Nurse to support MLTC enrollment. This field-based role centers on initial assessments, eligibility, safety, and planning care while guiding members and families through available services.

The ideal candidate has strong assessment and communication skills, and enjoys community-based work with older adults and complex healthcare needs.

Qualifications

• Active New York State RN license required.
• Minimum 1 year of acute care nursing experience.
• Home care and MLTC experience preferred.
• Knowledge of Medicare and Medicaid regulations.
• Willingness to travel 50%+ within territory.
• Bilingual skills (Spanish, Russian, Mandarin or Cantonese) a plus.

Responsibilities

• Conduct community outreach and intake activities as needed.
• Perform initial assessments using the UAS tool.
• Evaluate eligibility, safety, and care needs for enrollment.
• Develop an initial plan of care based on assessments.
• Explain program services to members and families.
• Coordinate with clinical, intake and outreach teams.
• Document enrollment and clinical information accurately.
• Serve as a community resource for intake questions.
• Consider age, developmental, mental, social and cultural needs.
• Maintain knowledge of Medicare and Medicaid requirements.
