Job summary

E & S Home Care Solutions, LLC in South Plainfield, NJ seeks a compassionate RN Field Nurse to perform in-home assessments and develop personalized plans of care. You will conduct initial assessments, supervise HHAs, and coordinate with families and office staff in a flexible, field-based role.

The ideal candidate holds an active NJ RN license, values independence, and can manage travel to clients’ homes with reliable transportation.

Qualifications

• Active and unrestricted RN license in New Jersey.
• Home care experience preferred.
• Strong nursing assessment and documentation skills.
• Excellent communication and organizational skills.
• Ability to work independently and manage field visits.
• Valid driver's license and reliable transportation.
• Flexible part-time and per diem scheduling.

Responsibilities

• Conduct initial nursing assessments and reassessments in clients' homes.
• Develop, review, and update individualized plans of care.
• Perform supervisory visits for HHAs.
• Evaluate clients' health conditions and identify changes in care needs.
• Ensure compliance with New Jersey home care regulations and agency policies.
• Complete all required nursing documentation accurately and within established deadlines.
• Communicate effectively with clients, families, caregivers, and office staff.
• Provide guidance and education to HHAs regarding client care needs.
