Job summary

Houston Hospice is seeking an RN Field Case Manager to oversee comprehensive case management for patients and families in the Houston area north of I-10. You will coordinate care with RNs, LPNs, physicians, and community partners, maintain documentation, and develop care plans; ensure compliance with state regulations and timely physician/psychiatrist involvement after admission. Prior case manager experience preferred.

Qualifications

• Maintain case documentation per state regulations.
• Develop and maintain individualized case plans.

Responsibilities

• Coordinate care with physicians and other providers.
• Maintain case documentation per state regulations.
• Develop and maintain client case plans based on assessments.
• Ensure timely visits by a primary physician or psychiatrist within 24 hours of admission.
• Provide home visits to optimize outcomes.
• Review charts weekly for follow-up and referrals.
• Coordinate connections with community agencies.
