Qualifications

• Licensed Registered Nurse in Texas (RN)
• 2 years of nursing experience required
• Hospice experience strongly preferred (2+ years)
• Valid driver’s license and reliable transportation

Responsibilities

• Meet with patients and families to explain the scope of hospice services and coordinate admission plans.
• Coordinate admission of patients and ongoing plan of care.
• Assess patient physical condition and revise Plan of Care as needed.
• Provide emotional support to patients and families and instruct in activities of daily living.
• Complete daily paperwork and charting according to policy.
• Evaluate hospice appropriateness and communicate findings to the Case Manager
