Job summary

21st Century Home Health Services in the San Francisco Bay Area is hiring an entry-level Field Registered Nurse to develop foundational clinical skills in home health while learning operational goals. This role requires visiting patients in their homes and works under the Case Management Registered Nurse to deliver skilled nursing care.

You will participate in care planning, complete timely documentation, and engage in weekly case conferences.

Qualifications

• One year of recent nursing experience (exception for internal candidates or new grad posting).
• One year of home health care experience is preferred.
• Experience with high acuity patients is a plus.
• Graduate of an accredited school of nursing (Bachelor's degree).
• Current CA Registered Nurse License (RN).
• Current Basic Life Support (BLS) Card.
• Valid CA Drivers License with acceptable driving record.
• Self- directed, Critical Thinker, Outstanding interpersonal skills, Organized, Computer literacy.

Responsibilities

• Provides hands-on patient care within RN license scope and plan of care.
• Implements a plan of care initiated and directed by the Case Management RN.
• Provides accurate and timely documentation within 24-48 hours.
• Assesses patient and family education needs relevant to diagnosis and plan of care.
• Coordinates home health services and reports needs to Case Management RN.
• Attends weekly Case Conferences to coordinate patient care information.
