Job summary

21st Century Home Health Services (21HHS) is seeking an entry-level Field Registered Nurse to develop foundational nursing skills in a home health setting and learn the company’s operating goals and efficiencies. The role requires field visits across Napa county and works under a Case Manager RN.

The position hires for full-time schedules (Wednesday through Sunday visits) with opportunities to grow in case management and clinical skills within a supportive team.

Qualifications

• One year of recent nursing experience (home health preferred).
• Graduate of an accredited school of nursing (Bachelor's degree).
• Experience working with an interdisciplinary team is highly desired.

Responsibilities

• Provide direct patient care within RN scope in a home health setting.
• Assist in implementing and documenting the care plan.
• Participate in case conferences and coordinate with Case Management RN and other disciplines.
