Job summary

21st Century Home Health Services in the San Francisco Bay Area seeks an entry-level Field Registered Nurse to develop fundamental nursing skills within a home health setting while visiting patients in their homes. It serves as a foundational role for RNs aspiring to advance into case management.

This position works under a Case Manager Registered Nurse, with CA RN license and BLS required, and a wage range of $130,000–$160,000.

Qualifications

• Graduate of an accredited nursing program (Bachelor's degree).
• Current CA RN license and BLS.
• Home health experience preferred.
• Interdisciplinary team collaboration experience.
• Valid CA driver's license required.
• Strong patient care, communication and documentation skills.

Responsibilities

• Provide hands-on patient care within RN scope, including wound care and procedures.
• Develop and implement a plan of care with Case Management RN.
• Document care within 24–48 hours according to the plan.
• Educate patients and families on diagnoses and home health care.
• Coordinate with HHA, OT, PT, MSW and other disciplines.
• Attend weekly Case Conferences and participate in on-call duties as required.
